AWARD CATEGORY APPLICATION FORM 2026
The deadline for submitting applications is 12 noon Tuesday 30th June 2026. Please expect an update on your application by 28th August 2026. If you have any questions please email hba@hartlepool.gov.uk or call 01429 363150.
Please note when completing your application:
- Before completing the application form please read the Award category Information and Application Criteria document which includes the Terms and Conditions of Entry.
- You must complete the application in one go, you cannot save and return to it later.
- Please refer to the specific category criteria when applying for an award.
- If you are applying for two awards then you can complete a single application, however you must refer to each category criteria.
Please Note: If you wish to nominate a person for The Peter Olsen Award you will need complete a separate nomination form.
